Confidential Ryan White Case Management Services

Personalized support to navigate healthcare and social resources, ensuring clients receive comprehensive care.

Initial Contact

Clients may initially interact with WCAY via walk-in or phone call or a partner agency may call to refer a client. Once we get contact with the client, we share what required documents they need to bring to the appointment (photo ID, proof of residency, proof of income, and proof of insurance if applicable).

WCAY Case Managers

prioritize getting clients into care with 72 hours of diagnosis. We Schedule visit with Primary Care Physician for HIV Treatment and Case Manager on the same day.

The Client

will complete required intake a clinic and see the Physician. Then, the client reports to the Case Manager appointment. The Case Manager will complete the Intake Assessment and understand client needs

Case Management Services

Medical Case Management

Case Managers schedule and/or track doctor appointments and support clients stay in care by eliminating barriers through medical transportation services, and incentivizing participation in care through Health Models Plus +, discuss medications and medication management.

Non-Medical Case Management

Case Managers assess needs, provide the services we can, and refer to community organizations who provide the needed services.
